Referrals

A clear pathway from enquiry to placement

Lighthouse accepts referrals from schools, Local Authorities and other agreed professional routes. Every placement is considered individually.

Process

Six steps to placement

  1. 1

    Initial enquiry

Contact us with a brief outline of the young person and the reason for referral.

  2. 2

    Referral form

Complete the referral form and share supporting documents.

  3. 3

    Information review

We review the information alongside safeguarding and risk context.

  4. 4

    Suitability decision

We confirm whether needs can be met safely and purposefully.

  5. 5

    Planning meeting

Placement is planned with commissioners, family and the young person.

  6. 6

    Placement start

Learning begins with a personalised, phased plan.

Information required at referral

To assess suitability we need a full picture of the young person and their context.

  • learner details
  • reason for referral
  • current educational setting
  • attendance history
  • SEND and EHCP information
  • safeguarding context
  • social care involvement
  • behaviour and risk
  • medical needs
  • allergies and medication
  • communication needs
  • transport arrangements
  • desired outcomes
  • current professional involvement

Admission principle

Lighthouse will only accept a placement where the learner's needs can be met safely, purposefully and within the agreed commissioning, staffing, risk and premises arrangements.
